Care and Support Plans
The process normally involves two appointments. The first appointment will cover any tests or checks that are needed and the results will then be available for the second appointment.
In the appointments you and the Nurse will be able to discuss any concerns or questions that either of you have. You will be look at what support you may want or need and if there is anything you can do to stay well. You and the Nurse will then work out a plan.
